Cancellation Policy:  There is cancellation charge equivalent to the full
    fee for appointments canceled with less than 24 hours notice.  If you
    cannot keep your appointment please let us know as soon as
    possible.  

    Payment is due at time of service.  Accepted forms of payment are
    cash, check, MasterCard, Visa, and debit or Health Savings Account
    cards with Visa or Mastercard logos only.

    Sliding Scale Acupuncture Fee Schedule
    Please read completely before determining your fee.

    Initial Fee:
    Please note your first visit is a lengthy visit involving examination,        
    assessment and treatment. The fee for your first visit is $130.  All
    follow-up visits are at sliding scale fee rates from $60 to $100 as
    determined by the Four Steps listed below.

    Follow-up Treatment Fee
    Your follow-up treatment fee is determined by considering your           
    assets, liabilities and gross annual income and not by gross annual       
    income alone.  

    Please follow Steps One through Four in calculating your
    fee.

    Step One:  Take into consideration trust funds, savings and assets,   
    alimony and other monies that are not part of your gross annual         
    income level that increase your ability to pay at a higher level.

    Step Two:   Now take into consideration illness, unemployment,
    child care, and other factors that may negatively impact your
    finances.   

    Step Three:  Your gross annual income should be considered along
    with information from Steps One and Two above and not used alone
    to calculate your fee.   Think about your income in light of your
    additional assets as well as your liabilities. It is all three of these
    factors that contribute to assessing your fee.

    Step Four:  Your fee is based on the honor system. No
    documentation need be provided.  We only ask that you consider all
    aspects of your finances when you determine your fee.  Your fee
    should be within the range of $60-$100.  Please be prepared to state
    your fee for follow-up visits at your initial acupuncture visit.

       Chinese Herbal Remedies

  •        If included WITH acupuncture treatment or Chi Nei Tsang:

    You will only be charged for the herbal formulas.  Price of herbal formulas varies
    based upon source, preparation and ingredients.

  •        Consults and recommendations WITHOUT acupuncture or Chi Nei Tsang
    treatments:

    There is a $125 fee for the initial consult.  
    Herbal formulas may or may not be determined or provided at the first session.

    Follow-up herbal consultations appointment are $50 for 30 min.
    You will also be charged for the recommended herbal formulas price of herbal
    formulas varies based upon  manufacturer, preparation and ingredients.

    Additional Information regarding fees:

    Please be aware that it may be decided during the initial consult that due to your
    condition or  medications you may be taking that herbal formulas may not be
    the  appropriate or best course of treatment for you at this time.  In that case the fee
    for the consultation may be reduced.

    If you have not been seen at the clinic for 3 months or more and a new intake is
    needed for Chinese Herbal Formulas there is a $75 re-evaluation charge.
